Sadly, we know that few general healthcare professionals have the specialist knowledge or experience needed to appropriately care for SCI people. That’s why we help people create personalised care plans that effectively communicate their unique care needs. We can help you create:
- A personal care plan. This document outlines your typical daily care needs and is helpful if you are going through a care assessment. You can print this from our website here to complete yourself or with the help of a family member or carer.
- Your emergency care plan (ECP). This is a potentially life-saving document that is completed by one of our SCI nurse specialists because it needs clinical validation. Your emergency care plan details your essential SCI care needs considering bladder, bowel, skin and respiration management and, where appropriate, autonomic dysreflexia. If you are admitted to hospital in an emergency, your plan will let the clinical team treating you know precisely what it is you need to stay safe and well.
Contact our SCI nurse specialists here to get your personalised emergency care plan.
